Supplemental Compensation Form (Classified Staff)
NOTE: This form can be printed on regular paper.
The Supplemental Compensation form for
Classified Staff is used to authorize compensation beyond the employee's
regular assignment. Please refer to Classified Staff Personnel Policy
Manual, 316.0 Supplemental Compensation for the definition of Classified
Staff work assignments that qualify for Supplemental Compensation.

Several of the fields on this form are self
explanatory. A basic overview of the form is provided below.
- Indicate if the request is Original, Revision (revising an existing request), Continuation (extending an existing request).
- Primary Position Information: Complete the Employee Name, etc. with the
employee's current employment information, as defined in PSOS.
- Supplemental Compensation Information: Complete with the proposed supplemental compensation
