Supplemental Compensation Form (Graduate Assistants/Associates)


Purpose

    The Supplemental Compensation form is used to authorize compensation in addition to the assignment currently held by the Graduate Assistant/Associate. If you have any questions, please contact the Julie Treanor or Rachel Zapp in the Graduate College at 621-5193.

Instructions

    Several of the fields on this form are self explanatory, those needing explanation are explained below:

    1. Indicate if the request is Original, Revision (revising an existing request), Continuation (extending an existing request).

    2. Primary Position Information: Complete the Employee Name, etc. with the students's GA Appointment or Summer Instructor position information. This is available from PSOS, screen 03.

      NOTE: If the student isn't in a GA Appointment or Summer Instructor position, they cannot be paid on a Graduate Assistant/Associate Supplemental Compensation form.

    3. Supplemental Compensation Information: Complete with the proposed supplemental compensation information.

Approvals

Obtain the following signature:

  • The Department Head of the department requesting supplemental comp.

    Send the signed original to Systems Control for processing. Retain a copy for your records.
