The Supplemental Compensation form is used to authorize compensation in addition to the amount specified in the Notice of Appointment or Personnel

- Indicate if the request is Original, Revision (revising an existing request), Continuation (extending an existing request).
- Complete the Employee Name, etc. with the employee's current
employment information.
- Supplemental Comp. Info: Complete with the proposed supplemental compensation information. Payperiod dates are available
